checked some more, used the german version which came with a preconfig version of dosbox in d-fend.

had to change soundblaster to SB2.

Other than that it works fine in 0.74.

So the issue seems not to be Dosbox but the game version which can be found on most sites.

The sound works fine with the german version of the game.
It offers sound blaster as a choice when starting the game.
Guess the sound issue is with an older english version? Start menu definitely looks updated.

Not really a fix but a choice.. no sound or different language.

It works fine except the sound area. (even running it without dosbox, but more speedy)

I tried to choose between all sound modes at the start menu of the game but nothing worked. :S even when emulating with vdms sound doesn't work... If anyone figure this out just reply :)
