DOSBox by default assumes that if no change to PC speaker state occurs for 10 seconds if output enabled, or 1 second if output disabled, and the output happens to be zero at the time that decision is made, the PC speaker output stops rendering.
There is a fix for this issue integrated in DOSBox-X.
Also, be sure to set cycles in the [cpu] section to a low value, such as cycles=350 as the game will otherwise run too fast.

When you play several times, you will see that this game sometimes crash with the following message: "Error in collision detection". This not happens if you play directly under pure DOS. Other issue is that this game runs too fast and if you drop down the cycle counting by the half (approximately 900 cycles) it will play at normal speed but it runs with a LOT of flickering, even with frameskip set to 0. And my machine is a P4 2.4Ghz.
