The game has support for stereo (separate sounds played though each channel), with the right setup.
Configure DOSBox to use a Sound Blaster Pro 1 or 2 (e.g. sbtype=sbpro1).
Then run SETSND.EXE and select option 3 "Sound Blaster Pro"
With vanilla DOSBox you may also get stereo sound with the default SB16 emulation, but that is actually because vanilla DOSBox does not emulate the SB16 properly. With a real SB16 you would not get stereo sound. For some DOSBox forks with more accurate emulation, this will not work.
Also as mentioned by madminne2, be sure to set cycles=5000 as otherwise the game will slowdown during busy scenes.

1. Get the latest Daumcafé-Version of DOSBOX (0.74 at this moment)
2. at [sdl], set :
fulldouble=true (setting fulldouble alone isn't enough to overcome tearing, read on)
fullresolution=1024x768 (my native res. 1280x1024 didn't do, only 1024x768 or less gave a REAL fullscreen without black bars)
output=ddraw (tried them all, on my old 5700 FX Ultra this works best)
3. at [dosbox], set
memsize=32 (see post below)
4. at [render], set
aspect=true (if you get a read fullscreen with your native resolution as described above you can leave aspect=off)
5. at [vsync], set
vsyncmode=host
vsyncrate=60 (AFAIK the host-mode reads the vsyncrate directly from the OS on a windows-based machine, but I put in 60 since I use a TFT monitor).
6. at [cpu], set
cycles=5000 (works for me on an old Athlon 64 3000+, 1 gig of RAM and WinXP SP3)
The overall result will be a much smoother, eye-friendly gameplay; it is really worth the short work.

you need to set mem=33 or lower otherwise the game wont start and show a xms allocaton error
